Scope of Appointment Form

Medicare Scope of Appointment (SOA) Process

What is the Scope of Appointment (SOA)?

The Scope of Appointment (SOA), a document mandated by the Centers for Medicare and Medicaid Services (CMS), is an essential part of the Medicare enrollment process. It ensures that any discussions between you and a Medicare agent are strictly limited to the topics you have consented to beforehand. This form helps protect your rights by preventing agents from promoting products or services that you did not express interest in. When you complete an SOA, you are simply outlining the specific Medicare plans or options you wish to learn more about. It’s a safeguard designed to create a transparent and focused conversation about your healthcare needs.

Why is it Important?

It protects you from unwanted information and aggressive sales tactics. It ensures agents follow Medicare guidelines and rules of conduct.

When Do You Need an SOA?

Required for one-on-one meetings, whether in-person or by phone, primarily concerning Centers for Medicare and Medicaid Services (CMS)-regulated Medicare plans (e.g., Medicare Advantage, Part D) only.

How to Complete the SOA Form (PDF) - See bottom of page for downloadable SOA Form.

1. Fill in Your Details

  •    Name and contact information.

2. Appointment Details

  •    Date of the appointment.
  •    Agent's name and contact information.

3. Topics to Discuss

  •    Check the topics you agree to discuss.

Important Points

  • You must sign the SOA Form at least 48 hours before the appointment (unless it's a walk-in or close to the enrollment period deadline).
  • The Form is valid for 12 months.

Penalties for Agents

  • Agents face fines and license issues if they don't follow the SOA rules and fail to obtain a properly completed SOA Form.

Next Steps

  • Download, fill out, and sign the SOA Form.
  • Return it to your agent at least 48 hours before your meeting.

Ensuring Compliance and Understanding

By following these steps, you ensure a safe and compliant Medicare discussion. Additionally, make sure to read the Scope of Appointment (SOA) Form Terms of Use & Disclaimer section(s) below on this page and website to fully understand the guidelines and requirements for your Medicare consultation.
